The nurse/case manager will ensure compassionate, high-quality home-based care for elderly clients by coordinating services, supervising caregivers, and upholding professional standards. She will serve as a vital link between clients, families, caregivers, and external partners; monitors service quality and compliance; mentors caregiving staff

Job Description/Requirements
Key Responsibilities:
- The operational and functional management of the Caregivers (CGs).
- Overseeing the quality of the services in accordance with applicable laws and regulations and the organisational mission.
- Answering and processing client requests, exploratory meetings with clients and their families, intakes and supervisory meetings at clients’ homes.
- The overall care service coordination during the period of care is given at the client’s home.
- Building and maintaining contacts with relevant network parties, so that the growth ambition of the organisation can be realised.
Qualifications:
Candidates must hold a bachelor's degree or diploma in nursing, registration with the Uganda Nurses and Midwives Council, and at least 3 years of working experience as a registered nurse; experience in working in home care of the elderly is an added advantage.
